Individualized Community Living Support

What is ICLS?

Individualized Community Living Supports (ICLS) is a service designed to help older adults remain in their own homes by providing individualized assistance based on their assessed needs.

Minnesota DHS describes ICLS as a bundled service with six service categories. It provides support to older adults who may need reminders, cues, intermittent or moderate supervision, or physical assistance to remain in their own homes.

ICLS is particularly focused on maintaining a person's ability to live safely and successfully in their own home and community.

Aging doesn't necessarily mean someone needs to leave the home they love. Sometimes an individual simply needs the right amount of support to continue living there safely. That could mean someone checking in during the day, helping with a routine, providing reminders, assisting with certain physical tasks, or supporting participation in everyday activities. Our goal is to meet people where they are and provide support that fits naturally into their lives.

Why Present Help?

We understand that maintaining independence means different things to different people. For one client, independence may mean continuing to prepare their own meals. For another, it may mean remaining in the neighborhood they've lived in for decades.

For someone else, it may mean being able to continue attending community, cultural, or faith-based activities.

We build our support around the individual's life—not the other way around.
