Our services may be appropriate for individuals who need ongoing support due to age, disability, chronic conditions, or other circumstances that affect their ability to independently manage daily activities.  Depending on an individual’s assessed needs and authorization, services may include:

 

1. Personal Care Assistance (PCA) — Assistance with eligible personal care and daily living needs.

2. Homemaking — Support with household activities and maintaining a safe and healthy home environment.

3. Companion Services — Social support, companionship, and assistance with activities that promote community involvement and quality of life.

4. Individualized Home Supports (IHS) — Individualized assistance and support designed around a person's needs, abilities, goals, and level of independence.

5. Individualized Home Supports with Training (IHST) — Support that may include skill development and training to help individuals increase or maintain independence.

6. Individualized Community Living Supports (ICLS) — Assistance and skill-building designed to help individuals live as independently as possible and participate in their community.

7. Night Supervision — Overnight support and supervision for individuals who have an assessed need for this service.

8. Other approved 245D services — Services may be available based on the individual’s assessment, support plan, authorization, and the services our agency is licensed and authorized to provide.
